Application for the Transportation Services Program

This is the Application for Keep Hope Alive USA’s Mental Health Services Program. To be eligible for this program you must complete this application in its entirety. You must be a foster child, fostering family member, or have a child in the foster care system to be eligible for this program. KHA USA will process your application as quickly as we can. In most instances your application will be processed within 24 to 48 hours during the business week. Please allow 5 business days before a KHA USA Staff member can get in contact with you.

Important Notice :Nonprofit Mental Health Service Limited Liability First and foremost, we want to assure you that our organization takes safety and quality of service very seriously. We have implemented rigorous selection processes, training programs, and operational protocols to minimize the risk of incidents and promote a culture of accountability and responsibility among our Social Workers/Specialist and affiliates.

However, despite our best efforts, unforeseen circumstances or human errors can occur. We want to make it explicitly clear that our nonprofit clothing procurement service/program cannot be held liable for any misunderstandings, safety issues, damages, losses, or injuries arising from the negligence, acts, or omissions of our designated stores, staff members, or any other affiliates associated with our organization.

We strive to foster a collaborative clothing service including all stores, members, and partners, share the responsibility for ensuring a safe and efficient clothing service experience. We encourage you to take necessary precautions, such as being vigilant of the process, adhering to safety guidelines to acquiring clothing, and promptly reporting any concerns or incidents to our organization.

Please note that we are dedicated to investigating and addressing any reported issues promptly and taking appropriate actions to rectify them. We value your feedback and are committed to continuously improving our services.

By continuing to avail yourself of our clothing procurement services, you acknowledge and accept that our nonprofit organization cannot be held accountable for the actions, negligence, or any other shortcomings of stores/facility or affiliates.
